We invite you to attend an upcoming substitute orientation event.

Attached to this email are the Substitute New Hire forms. (Click Here For Forms) . Please only complete the forms if you intend on attending the substitute orientation event.
· In addition to completing the D76 forms that have been shared with you, you are asked to provide the following:
o Current Educator Licensure Information System print out from ISBE.net
o Copy of current Physical Exam & TB Test Results (must be within the last 90 days)
o Two ORIGINAL forms of identification as listed on the Employment Eligibility Verification form
· The completed forms are to be returned in person at the Substitute Fair being held on Wednesday, September 14, 2016
9:00 AM – 12:00 PM.
· Upon submitting the completed forms and required documents, you will be provided with a form to take to the Mundelein Police Department to have an electronic fingerprint / background check completed. This form is available for you to print from the Substitute New Hire forms shared with you in this email.
